To download the form:

  • Download a copy of the claim form in PDF format.
  • Print the claim form.

To complete the form:

  • Be sure to take your pet to a licensed veterinarian. You and your veterinarian complete the relevant parts of the claim form.
  • Pay the veterinarian bill in full and have your veterinarian complete sections 2 and 3 of the claim form.
  • Complete sections 1 and 4 of the form and be sure to sign it.
  • Be sure to attach the original veterinarian bill or receipt.
  • Once the form is completed, mail it to us at:

Tips for your veterinarian:

  1. Please fill out Section 3 to the best of your knowledge.
  2. Please indicate the diagnosis or tentative diagnosis, rather than "x-rays, blood work, etc…"
  3. If the condition is not known, please specify clinical signs and symptoms. (e.g. fever, lethargy, weight loss, etc.)

Things to remember:

  1. Always retain a copy of your claim form and receipts for your records.
  2. Include copies of all receipts and invoices.
  3. Use one claim form per pet.
  4. Issuance or completion of this form does not acknowledge liability on behalf of SecuriCan.
  5. There are time limitations on submitting claims. Refer to your Policy Terms, Conditions, and Benefits of Insurance document for more details.
  6. Claim forms that are not properly completed may be delayed or returned to sender.
  7. Deliberate misrepresentation of your pet's condition or the omission of any material facts may result in denial of the claim and/or cancellation of the policy.
